Pines Nursing & Rehab is seeking a dedicated and compassionate Psychiatric Nurse Practitioner to join our long-term care team in Easton, Maryland. We are looking for a provider who values a smaller, family-oriented environment and is passionate about improving the mental health and quality of life of our residents.

The Psychiatric Nurse Practitioner will provide comprehensive psychiatric care to residents within our skilled nursing and long-term care facility. This role involves conducting psychiatric evaluations, developing individualized treatment plans, managing psychotropic medications, and collaborating closely with interdisciplinary teams to support residents with behavioral health and cognitive needs.
 
Responsibilities
• Conduct comprehensive psychiatric assessments and evaluations for long-term care residents
• Develop, implement, and monitor individualized psychiatric treatment plans
• Provide medication management, including prescribing and monitoring psychotropic medications
• Collaborate with physicians, nursing staff, social services, therapy teams, and families to coordinate care
• Monitor residents for behavioral changes, medication effectiveness, and potential side effects
• Assist with behavioral interventions and crisis management when needed
• Maintain accurate and timely medical documentation in compliance with facility and regulatory standards
• Educate residents, families, and staff regarding mental health conditions and treatment approaches
• Participate in care planning meetings and interdisciplinary team discussions
• Support a compassionate, resident-centered approach to behavioral health care within the facility

 
Qualifications
• Current and active Nurse Practitioner license
• Psychiatric Mental Health Nurse Practitioner (PMHNP) certification
• Experience in psychiatry, behavioral health, geriatrics, or long-term care preferred
• Strong understanding of psychotropic medications and behavioral health management
• Excellent communication, clinical assessment, and documentation skills
• Ability to work independently while collaborating effectively with an interdisciplinary team
